Please Note , the price on the financial bid form must be the same with the bid price. In the case of different prices the price o the financial bid price will be considered

1. the Bid Submission Sheet in this Part;
2. a copy of every document necessary to demonstrate eligibility in terms of section 28 (1) of the Regulations;
3. Supplier Registration number showing that you are registered with the Procurement Regulatory Authority of Zimbabwe;
4. Authority of Zimbabwe (PRAZ)
5. Valid VAT certificate
6. ZIMRA valid Tax clearance (ITF 263)
7. NSSA valid registration certificate
8. Certificate of incorporation, CR14, CR6 and CR5
9. A Bid Securing Declaration in the format specified in Part 2
10. Company Profile showing similar supplies performed in the last 3 years
11. Submit at least three (3) reference letters with contact details of referees where previous supplies have been performed.
12. 4 Months STAMPED bank statement. 
13. A signed declaration showing that there is no conflict of interest in relation to this procurement requirement
