You are requested to bid for the items described in the Statement of Requirements below, by completing and returning the following documentation:   

1. The Bid Submission using a form from this document.    
2. Provide CR5 forms 
3. Provide CR6 form
4. Provide CR14 form
5. Provide Valid Tax Clearance Certificate (ITF 263)   
6. Proof of Registration with Insurance and Pension Commission (IPEC) 7. Proof of Membership with Insurance Brokers Association of Zimbabwe (IBAZ)   

8. NSSA Compliance Certificate.
9. PRAZ registration certificate of the appropriate category.  
10. A copy of Company Profile.   
11. Provide at least three (3) traceable references for the provision of insurance services.
12. Declaration of conflict of interest document.
13. Bid securing declaration form using a form from this document.
14. Priced BOQ
15. Site visit certificate  (Compulsory Pre bidding meeting)  

You are advised to carefully read the complete Bidding Document, as well as the General Conditions of Contract which are available on the Authority’s website, before preparing your Bid.  The standard forms in this document may be retyped for completion but the Bidder is responsible for their accurate reproduction.  All pages of the Bid must be clearly marked with the Procurement Reference Number above.
