4. State Quotation [Bid] validity [MINIMUM 30 DAYS AFTER CLOSING DATE]  
5. Clearly state the delivery period.  
6. Attach valid proof of registration with PRAZ ON RELEVANT CATEGORY.  
7. Past performance will be considered during the evaluation.  
8. Attach CR14/CR6 (List of Directors)
